Creates a questionnaire for users.
To conduct a questionnaire, first create master data such as the questions on the "Edit Questionnaire Template" screen, then specify the target respondents and response period in "Questionnaire Group Management" to actually conduct and compile the results.
You can also conduct the same questionnaire multiple times using one questionnaire template.

Click the "Create New" or "Edit" button to open the questionnaire template editing screen.
Enter the following items and click the "Update" button to create/update the questionnaire template.
You can copy an existing survey template.
The template, questions, and question options will be copied.
Create the questions and answer options to be included in the survey.

This is an example of the settings in the master management screen and the screen where the answers are actually displayed. (When "Multiple Answers" is "No")
If you do not create answer options, you can conduct a survey where only comments can be entered.
